Report released by MOHLTC Avoidable Hospitalization Advisory Panel, chaired by Dr. Ross Baker
IHPME faculty and graduates played a lead role on a provincial panel and new report focused on avoiding hospitalizations in order to improve quality of care and the sustainability of the health system.
The Ministry of Health and Long-Term Care established the Avoidable Hospitalization (AH) Advisory Panel in September 2010. It was asked to identify:
- system-wide AH best practice
- form and content of an AH improvement practices inventory; and
- measures and an evaluation framework for AH initiatives.
The Panel’s report, Enhancing the Continuum of Care, is now available online.
The Panel was chaired by IHPME faculty, Ross Baker, and included faculty members Walter Wodchis, Chaim Bell and Irfan Dhalla. Several IHPME graduates also served on the panel: Bill MacLeod, CEO, Mississauga Halton LHIN; Paula Blackstien-Hirsch, Senior Director, Ontario, Canadian Patient Safety Institute; Patti A. Cochrane, Vice President, Patient Services & Quality and Chief Nursing Officer, Trillium Health Centre; and Emily Lap Sum Musing, Executive Director of Pharmacy, Clinical Risk and Quality and Patient Safety Officer for the University Health Network.
