Primary Health Care: Present & Future

Primary care providers are a key entry point into the health care system and are often responsible for coordinating care over time and across sectors. In the last decade, innovations in primary health care delivery have been implemented across Canada. Ontario is a leader in the introduction of a variety of new initiatives including funding models, 24/7 care, interdisciplinary teams, electronic medical records and quality improvement initiatives from health promotion/illness prevention to complex chronic care. This seminar series will explore the benefits and challenges associated with the implementation of PHC models in Ontario and other jurisdictions.
