Accommodation

Accommodation is available at the BMO Institute for Learning. A block of rooms is being held. Cost is $185.00 per night plus applicable taxes in addition to the registration fee. No refund for cancellations within 3 days of program start. 

Please indicate accommodation requirements on the registration form. You will be invoiced separately for accommodation.

Note : Accommodation must be booked and prepaid by August 12, 2012.

No. of Participants

The University maintains program effectiveness by limiting the program numbers to a maximum of 35 participants and requires a minimum of 20 participants.

Tuition

Program tuition includes all study materials, continental breakfast and lunch, and dinner where appropriate.

Discounts
There will be a discount on tuition ($4,000.00) only for multiple participants from the same organization:

                            2-3 Participants - $3,625.00 per person
                            4-6 Participants - $3,225.00 per person
                            7+ Participants -  $3,025.00 per person

Please note that the amount of $25.00 is the registration fee for Continuing Education.

In Event of Cancellation

In the event of cancellation before the program begins, tuition will be refunded less any requirements for hotel commitments and a $150.00 administration fee. A 50% refund will be issued for cancellation within 4 days of the beginning of the program unless a substitution is arranged.

Accreditation

This program meets the accreditation criteria of the College of Family Physicians of Canada and has been accredited for 38.5 MAINPRO-M1 credits.

This event is an Accredited Group Learning Activity (Section 1) as defined by the Maintenance of Certification program of the Royal College of Physicians and Surgeons of Canada, approved by the University of Toronto (38.5 credits).

The Office of Continuing Education and Professional Development, Faculty of Medicine, University of Toronto, designates this educational activity for a maximum of 38.5 Category 1 credits toward American Medical Association Physician's Recognition Award.
